NBM HCH rollout
The Health Care Homes model has been successfully adopted in US, Canada, UK and New Zealand.
In Australia, 10 Primary Health Network regions have been selected representing all States and Territories to roll out Phase 1 of the Health Care Homes (HCH) Model. The Nepean Blue Mountains (NBM) rollout is due to commence in early December 2017.
Evaluation
A comprehensive evaluation of the model has been commissioned by the Department of Health and an interim report will be provided to Government in early 2019 and a final report in June 2020. Evaluation results and lessons learned from the evaluation will be used to make refinements to the Health Care Homes model and inform any future national roll out.
The aim of the evaluation is to:
- identify changes in the way participating practices organise and deliver health care to their patients
- estimate early impacts of the Health Care Homes model on patient outcomes
- assess the suitability of Health Care Homes for national rollout for different practice types across a range of contexts
Purpose of Health Care Homes Initiative – Quadruple Aim of Health
The Quadruple Aim of health forms the foundation for the Health Care Homes Model as follows:
- Improved patient experiences of care
- Improved health outcomes and population management
- Improved cost efficiency and sustainability in health care
- Improved health care provided experience.
What support will be available to practices to implement the HCH Model?
- A dedicated Health Care Homes Practice Facilitation Team assigned to work with and support participating practices in their adoption of the Model;
- A suite of training and educational resources for practices, including comprehensive on line module;
- A National Practice Facilitator through AGPAL assigned to support the PHN Facilitators;
- Participation in a Learning Collaborative for Practice Facilitators and the creation of Communities of Practice to share ideas and learnings.
The Health Care Neighbourhood
A core component of the successful delivery of outcomes under the Health Care Homes Model will be the effective use of Shared Care Planning supported by implementation of the Healthcare Neighbourhood.
The Healthcare Neighbourhood incorporates the broader health system and social service system that can work in collaboration with the GP Practice to enhance the patient care.
