Diabetes Management Quality Improvement Collaborative

In 2022, approximately 24,000 Nepean Blue Mountains residents were living with diabetes. Diabetes is the fastest growing chronic condition in Australia, affecting the entire body and can have a significant impact on quality of life and reduce life expectancy. While there is no cure, diabetes can be effectively managed. Early diagnosis, optimal treatment and effective ongoing support and management reduces the risk of diabetes related complications.

What is a Collaborative?

A collaborative is a specific method of quality improvement. It supports participants in implementing change in small, manageable cycles and identifying where change actually leads to an improvement. This promotes rapid change, allowing participants to experience the benefits of their change activity and to create results in short time frames.

Collaborative Waves

The ‘wave’ of the Collaborative methodology commences with an Orientation Workshop followed by Baseline Data Collection. This baseline data is critical to understand the starting point of the collaborative, the program itself and the participants. The baseline data is presented at the first of the Learning Workshops. Each Learning Workshop is followed by an Activity Period.

During the Activity Period, the practices complete the Model for Improvement (MFI), which includes Plan-Do-Study-Act (PDSA) cycles. Step one of the MFI involved answering three key questions, which assist with developing a goal, measures for tracking the goal, and ideas that are tested to achieve the goal. Step two of the MFI involved selecting one of the ideas developed in step one, then planning, doing, studying and acting on the idea.

Expression of Interest

The Diabetes Management Quality Improvement (QI) Collaborative is working with general practice to improve quality of life for people living with Diabetes and reducing preventable hospital admissions in our region. Expressions of interest are now open for general practices to participate in the collaborative.

Eligibility

  • Must commit two clinical staff to actively participate in the Diabetes Management QI Collaborative. Both staff will need to commit to attending the online orientation and face to face learning workshops.
  • Need to commit to submitting data that pertains to the measures of the collaborative.
  • Will receive free training and online resources for diabetes management.
  • Need to sign up to LUMOS
