Mobile Occupational Therapist - Expression of Interest

We are seeking an occupational therapists to deliver in-home assessments for older adults aged 65 years and over (or aged 55 years and over for Aboriginal and Torres Strait Islander peoples) residing in Lithgow. The occupational therapist will support new and existing clients, with referrals coming from local GPs and through My Aged Care.

Background

Access to timely in-home assessments for older adults in Lithgow can be challenging, with wait times experienced to access some services. Navigating the application process for My Aged Care can also be a difficult process for some, which has led to eligible older adults not being able to access the available support they need. 

Eligibility

  • Must reside in Lithgow
  • Hold relevant qualifications in occupational therapy (level 3 or 4), including evidence of current AHPRA registration
  • Have a current NSW driver licence
  • Be registered as a GEAT2GO provider, or be willing to apply. 

What's involved

The role involves delivering a 0.2 full-time equivalent (FTE) occupational therapy service until June 2026. Key responsibilities include:

  • Provide community-based occupational therapy services to clients aged 65 years and over (or 55 years and over for Aboriginal and Torres Strait Islander peoples) living in their own homes
  • Conduct initial in-home assessments and writing required reports
  • Assess the need for home modifications and equipment to support independent living
  • Make referrals to appropriate home modification and equipment providers
  • Assist clients to create My Aged Care referrals and liaising with the Regional Assessment Service to receive referral codes
  • Provide interventions of up to eight hours per client and completing required reports
  • Submit regular progress reports, data collection and a final evaluation report to our organisation.
