Exercise Physiologist / Case Manager

Wallerawang Surgery is currently seeking an Exercise Physiologist / Case Manager to join their multidisciplinary team.

This role will involve both providing treatment of a variety of conditions, exercise prescription, education and case management.

Key Responsibilities:

• Manage a varied case load
• Developing RTW plans in collaboration with stakeholders
• Conduct patient assessments and provide ongoing care coordination
• Work independently while effectively prioritizing and organizing daily tasks
• Collaborate with other healthcare professionals to deliver high-quality care
• Foster positive relationships with patients, their families, and colleagues
• Maintain up-to-date knowledge on best practices and engage in continuous professional development.

• Accreditation from Exercise and Sports Science Australia (ESSA).
• Current First Aid and CPR certifications
• Strong verbal and written communication skills and ability to build rapport with clients and colleagues
• Demonstrated ability to work autonomously, manage time effectively and work well as part of a team
• Passion for ongoing learning and career progression in holistic healthcare
• Experience in case management
