Expression of Interest: Connector Point in Pharmacy

We are inviting pharmacies in the Blue Mountains, Hawkesbury, Lithgow and Penrith local government areas to join our Connector Point in Pharmacy Program, aimed at helping older adults remain socially connected. Pharmacies can participate by completing training and engaging in social prescribing activities to support patients dealing with loneliness and social isolation. The EOI will close on 4 November 2024.
