What is a Healthcare Neighbourhood?
A Healthcare Neighbourhood is team of people that work together to meet your healthcare needs. This means, your GP, nurse, allied health professionals, social service providers, family and carers, and most importantly YOU will be involved in your care.
The aim of the Upper Blue Mountains Healthcare Neighbourhood Collaborative is to come together and share ideas about how we can strengthen local relationships in aged health. This will increase communication across the wider care team, to improve your experience of the health care system and health outcomes.
There are a range of benefits for patients whose GP is involved in the Healthcare Neighbourhood, including:
