My Health Record

My Health Record is a secure online compilation of documents and information related to a patient's health which can be viewed by registered healthcare providers and the individual across Australia.

Registered healthcare providers who are providing care to an individual can view and upload information to the person’s My Health record without seeking permission each time they access it (subject to any access controls that the individual may have in place).

Benefits of My Health Record:

  • In a medical emergency, healthcare providers connected to the My Health Record system can see their patient’s health information such as allergies, medicines and immunisations.

  • When a healthcare provider accesses a patient’s My Health Record, it reduces the burden on the patient to have to remember and repeat their medical information, such as prescriptions or names of investigations.

  • Can help patients keep track of their children’s health, immunisations and investigations.

  • Patients can have authorised or nominated representatives who can access records on behalf of those who need help to manage their own care.

My Health Record supplements existing health records with a high-value, shared source of patient information that can improve care planning and decision making.

Information you can access through My Health Record includes:

  • Clinical documents added by healthcare providers including:
    • Shared health summary
    • Advanced Care Directive
    • Goals of Care documents
    • Event summaries
    • Prescribed and dispensed medication records
    • Pharmacist shared medication lists
    • Medicines and allergy information
    • Hospital discharge summaries
    • Pathology and diagnostic imaging reports
    • Specialist letters and referral documents
    • Medicare and PBS information stored by the Australian Department of Human Services, Medicare and RPBS information stored by the Department of Veterans' Affairs
    • Organ donor decisions
    • Immunisation information recorded in AIR, or uploaded to documents in My Health Record
    • Consumer entered information uploaded by the patient or an authorised representative including:
    • Contact numbers and emergency contact details
    • Current medications
    • Allergy information and any previous adverse reactions
    • Indigenous status
    • Veteran or Australian Defence Force (ADF) status
    • Advance care planning documents
