Chronic Condition Management (CCM)
New Medicare Benefits Schedule (MBS) Items from 1 July 2025
MBS Online has released the Upcoming Changes to the Chronic Disease Management Framework, confirming the implementation of a new framework effective from 1 July 2025.
The updated framework aims to simplify, streamline, and modernise chronic disease management for both healthcare providers and patients.
To support a smooth transition, existing patients will continue to have access to services under current arrangements for a two-year period.
Key Updates
- Items for GP management plans, team care arrangements and reviews will be replaced by a single, streamlined GP chronic condition management plan (see table one below).
- MBS fees for planning and review items will be equalised ($156.55 for GPs, $125.30 for prescribed medical practitioners).
- Multidisciplinary team requirements have been simplified, removing the need to collaborate with two providers. Multidisciplinary care plan items (e.g. 231, 232, 729, 731) remain unchanged.
- Practice nurses, Aboriginal and Torres Strait Islander health practitioners, and Aboriginal health workers can now support plan preparation and review.
- MyMedicare registered patients must access chronic condition management plans and reviews through their registered practice. Other patients can continue using their usual GP.
- A GP chronic condition management plan can be prepared once every 12 months if needed and reviewed every three months. A new plan each year isn’t required - existing plans can continue to be reviewed.
- Patients with a GP management plan or team care arrangement in place before 1 July 2025 can keep accessing services under those plans for two years. From 1 July 2027, a new GP chronic condition management plan will be needed to continue allied health services.
