Connector Points

Connector Points are community organisations and services that can provide face-to-face or telephone assistance for improving social connections and reducing social isolation of older people, particularly those without internet access. You can access the community connector points on the My Health Connector website.

Pharmacy Connector Points are another way older people can access social connections. Pharmacies are an important service in the community who see patients who may be experiencing social isolation and/or loneliness due to a range of life circumstances. Pharmacists are in a unique position to assist patients.

Through this initiative, pharmacists conduct social prescribing, which involves connecting patients with non-clinical services, social groups, or lifestyle activities. It can be used to address a range of health issues, including both physical and mental health conditions. You can view these Pharmacy Connector Points on our My Health Connector website.

The Connector Point program can be found in Blue Mountains, Lithgow and Penrith LGAs and is ongoing in Hawkesbury. 

Pharmacy Addressing Loneliness Training Program (PALs)

Completing the PALs training course will give you confidence in assisting patients experiencing loneliness. You will be able to identify common causes and signs of loneliness, know how to have supportive conversations with people who are lonely about their social health needs, and help connect them to support in the community, giving advice or referring on where needed. You will also know how to address loneliness at the pharmacy and community level. Register now.
